Two declarations, and they are not interchangeable

Annex V of the Machinery Regulation has two parts, and they are different instruments rather than two formats of one document. Part A is the EU declaration of conformity for machinery and related products. Part B is the declaration of incorporation for partly completed machinery, and it carries a statement the other one does not: that the partly completed machinery shall not be put into service until the final machinery it goes into has itself been declared conforming. This free tool draws up either, verbatim in your language. Directive 2006/42/EC is repealed with effect from 20 January 2027 — but Article 52(1) protects products placed on the market under it before that day.

Which declaration

Which part applies follows from whether the product is machinery or partly completed machinery under Article 3 — that is your classification, not something this tool can read off the fields. Part B is for something that is almost machinery but cannot perform a specific application on its own, and it is declared for incorporation into something else rather than declared conforming in itself.

The annex says in its own footnotes that assigning a number is optional. Leave it empty and the heading prints without one.

Article 10(8) lets you ship the declaration with the product or instead give the internet address or machine-readable code where it can be accessed, in the instructions for use. The second route is not the lighter one: a digital declaration has to stay accessible online for the expected lifetime of the machinery and in any event for at least 10 years after it is placed on the market or put into service.
This draws up a declaration. It does not assess conformity, classify your product under Article 3, choose your conformity assessment procedure, or decide whether a notified body was required — Part A items 8 and 9 are the two routes and which applies turns on Article 25 and on whether the product is listed in Annex I. Item 6 names other Union harmonisation legislation, and which acts to name is your assessment. The declaration is signed by you and the responsibility for it is yours.
